A nutritionist's first session is half admin — what someone eats, what they have tried, what a doctor has told them — and it eats the hour that should be doing the work.

This template collects the practical part beforehand, on WhatsApp: what they want help with, whether a clinician has referred them, any allergies or intolerances, and how the packages work. It gives no dietary advice whatsoever, because that is the consultation, and it hands over immediately when someone describes a medical condition or an eating disorder.
